Adding Custom Forms to Calendars
Beyond just picking a date and time, you can attach a custom form to a calendar to collect additional information from the person booking — qualifying questions, special requests, or anything else you need before the appointment happens.
How it works
Once a form is attached, it appears after the booker selects a time slot and before the booking is finalized. Whatever they enter is saved automatically to their contact record in the CRM, so the information is available to your team right alongside the appointment.
If your form includes conditional logic, you can use it to disqualify certain leads before the appointment is even created — which also blocks any associated payment from being collected, so you never charge someone for a booking that shouldn't have gone through.
Setting it up
- Go to Calendars > Calendar Settings, and click Edit on the calendar you want to modify.
- Open the Forms & Payment tab.
- Select your form from the Select Form dropdown.
- Save your changes.
Things to know
- Only one form can be attached to a calendar at a time.
- The same form can be reused across multiple calendars, but you need to attach it individually to each one — attaching it to one calendar doesn't automatically apply it elsewhere.
- Data is only saved once the booker clicks the final Book Appointment button. If someone starts filling out the form and abandons the process before confirming, nothing is recorded.

A custom form turns your calendar into a lightweight intake tool — useful for qualifying leads, gathering details your team needs ahead of time, or meeting compliance requirements before a service is delivered.
