Adding Intake Forms to Individual Services
Different services need different information before the appointment happens. A haircut booking might need nothing more than a name and phone number, while a medical consultation needs health history. Attaching a dedicated intake form to each service means you only ask customers what's actually relevant to what they're booking.
Why service-level forms beat one generic form
A single form applied to every service on your calendar either asks too little for your complex services or too much for your simple ones. Service-level forms let you tailor the questions — and the length — to what that specific appointment actually requires.
Setting up a service-specific intake form
- Open the service you want to attach a form to.
- Find the intake form (or booking form) setting for that service.
- Either select an existing custom form or build a new one with the fields you need — text fields, multiple choice, file uploads, and consent checkboxes are common options.
- Mark which fields are required versus optional, since required fields block submission until filled in.
- Save and test the booking flow to confirm the correct form appears for that specific service.
Reviewing responses
Submitted intake answers are typically stored on the customer's contact record and visible alongside the appointment, so staff can review them before the appointment starts rather than asking the same questions again in person.
Good to know: Keep required fields to the minimum you truly need — every additional required field on an intake form is a small amount of booking abandonment, especially on mobile.
